12 week injury not logged in transactions

Player on my team shows Out (Ankle) - 12 weeks; but his transaction log does not show an ankle injury this year. It's week 8 of 2028, and his last injury was 2028 Pre-Season Game 4 for Neck. His last ankle injury was 2026.

When did the serious ankle injury occur?
